Documents Required for Business Accounts

 

Please bring required documents with you when you visit one of our locations to set up your Business Account. 



SOLE PROPRIETOR

Forms Required:
• Assumed Name Certificate
• Proper Identification and verification of all signers. (Copy of Driver’s License)
• Employer Identification Number (EIN) or Tax Payer Identification Number (TIN)
• Current Drivers license

CORPORATION

Forms Required:
• Articles of Incorporation / Certificate of Formation
• Assumed Name Certificate if the corporation is doing business
under a name other than the name of the corporation.
• Proper Identification and verification of all signers (Copy of Driver’s License)
• Employer Identification Number (EIN) or Tax Payer Identification Number (TIN)
• Meeting Minutes signed by all directors
• Certification of Authority for out of state corporations - if applicable
• Current Drivers license

PARTNERSHIP

Forms Required:
• Partnership Agreement signed by all partners
• Certificate of Limited Partnership (if Limited Partnership) / Certificate of Formation
• Proper Identification and verification of all signers (Copy of Driver’s License)
• Employer Identification Number (EIN) or Tax Identification Number (TIN)
• Assumed Name Certificate if operating under any name other than the exact name listed on the Partnership Agreement.
• Current Drivers license

LIMITED LIABILITY COMPANY

Forms Required:
• Operating Agreement signed by all members or managers.
• Articles of Organization / Certificate of Formation
• Proper Identification and verification of all signers (Copy of Driver’s License)
• Employer Identification Number (EIN) or Tax Payer Identification Number (TIN)
• Assumed Name Certificate if the LLC/LLP is doing business under a name other than the name of the LLC/LLP.
• Certificate of Authority for out of state corporations - if applicable.
• Current Drivers license

UNINCORPORATED ASSOCIATION

Forms Required for Unincorporated Association/Organization/Club:
• Association or club minutes authorizing the opening of an account or to change signers.
• Employer Identification Number (EIN) or Tax Payer Identification Number (TIN)
• Proper Identification and verification of all signers (Copy of Driver’s License)
• 501c 3 (ss4) letter determining non-profit status if applicable.
• Current Drivers license

PROFESSIONAL ASSOCIATION

Forms Required for Professional Organization:
• Certificate of Formation / Articles of Association
• Employer Identification Number (EIN) or Tax Payer Identification Number (TIN)
• Proper Identification and verification of all signers (Copy of Driver’s License)
• Meeting minutes or bylaws.
• Assumed Name Certificate if the association is doing business under a name other than the name of the association.
• Current Drivers license
